The settlement class includes all former and current enrollees of the Kaiser Foundation Health Plan who accessed certain Kaiser Permanente websites or mobile applications from November 2017 to May 2024.

Your Class Member Unique ID number can be found on the bottom of the postcard notice or on top of the emailed notice you received.
Request your Settlement Class Member Unique ID by providing your email address on the “Request your Settlement Class Member Unique ID” page of the Kaiser Privacy Breach Settlement website found here. You can also go to the Contact Us tab on the website, complete your information and select “I am requesting my Settlement Class Member Unique ID” as your Reason for Contact.
To be eligible for payment in accordance with the settlement, you must submit a valid claim form to the Settlement Administrator by March 12, 2026. Only claims sent to the Settlement Administrator will be eligible for recovery.
You must file your claim with the Settlement Administrator. To file your claim online, go to the website Kaiser Privacy Breach Settlement and click on the tab for the Online Claim Form. You have the option to submit the form online, or printing and sending it via mail.

SUBMITTING YOUR CLAIM FORM ONLINE:
- Enter your Class Member Unique ID at the bottom of the Online Claim Form page in the “Settlement Class Member Unique ID (Required)” section highlighted in yellow.
- Once your Unique ID is entered click the blue Next button and you will be taken to the online version of the form.
Complete the online form, click Next, then review and click Submit


If you are unable to file a claim online or download the printable version, you may request a Claim Form be sent to you via mail by contacting the Settlement Administrator via telephone at 1-855-783-3816 or via email at: info@kaiserprivacysettlement.com.
Enter the Class Member ID number at the bottom of the Online Claim Form page in the “Settlement Class Member Unique ID (Required)” section highlighted in yellow. Hit next to go to the fillable online form. The name of the class member associated with the Unique ID number entered will populate in the form. You can then move forward to completing the claim for this class member.
To identify the Class Member associated with the ID number you received: Enter the Class Member ID number at the bottom of the Online Claim Form page in the “Settlement Class Member Unique ID (Required)” section highlighted in yellow. Hit next to go to the fillable online form. The name of the class member associated with the Unique ID number entered will populate in the form.
To request additional Class Member ID #’s that you think you have not yet received for a spouse or child, please go to the Request your Settlement Class Member Unique ID section on the website and enter your email address. You can also go to the Contact Us tab on the website, complete your information and select “I am requesting my Settlement Class Member Unique ID” as your Reason for Contact.
Yes. When submitting on behalf of a deceased class member use the Online Claim Form, revise the class member name to the name of the beneficiary when submitting. You will receive your confirmation email which you will need to please reply to and attach: (1) A copy of the death certificate and any additional documentation showing the beneficiary’s entitlement to the assets (e.g. will, executor of estate, etc.); and (2) A letter with instruction specifying how payments should be made. For example, check payable to____, or electronic payment if that option was selected.
If submitting your claim by mail, fax, or email, the beneficiary should follow the same steps above, complete the claimant information with their name and attach the same documents and letter indicating the payment method with their submission.
Please notify the Settlement Administrator if you want to cancel your exclusion by using the Contact Us tab on the website or, email info@kaiserprivacysettlement.com. You will then need to follow the procedures to file a Claim Form (See How can I file a claim?).
Claims processing and payment is being handled by an independent Settlement Administrator, Strategic Claims Services, which was appointed by the Court and is not affiliated with Kaiser. Strategic Claims Services will not provide Kaiser with a list of Settlement Class Members who have, or who have not, submitted claims absent a Court order requiring Strategic Claims Services to do so. Kaiser will be notified of all individuals who seek to exclude themselves from the Settlement Class.
