To identify the Class Member associated with the ID number you received : Enter the Class Member ID number at the bottom of the Online Claim Form page in the “Settlement Class Member Unique ID (Required)” section highlighted in yellow. Hit next to go to the fillable online form. The name of the class member associated with the Unique ID number entered will populate in the form. To request additional Class Member ID #’s that you think you have not yet received for a spouse or child, please go to the Request your Settlement Class Member Unique ID section on the website and enter your email address. You can also go to the Contact Us tab on the website, complete your information and select “I am requesting my Settlement Class Member Unique ID” as your Reason for Contact.
