If our family received multiple notices to the same email address with multiple Class Member ID numbers, how do we know which ID number is associated with which family member?

Enter the Class Member ID number at the bottom of the Online Claim Form page in the “Settlement Class Member Unique ID (Required)” section highlighted in yellow. Hit next to go to the fillable online form. The name of the class member associated with the Unique ID number entered will populate in the form.  You can then move forward to completing the claim for this class member.
